What is Refractory GERD?
Refractory gastroesophageal reflux disease (GERD) is defined as persistent troublesome symptoms despite optimal proton pump inhibitor (PPI) therapy. According to Lyon Consensus 2.0, this specifically means:
- Symptoms persisting after 8 weeks of optimized PPI therapy
- PPI taken 30-60 minutes before meals
- Adequate dosing (once or twice daily as needed)
Critical Definition
Before labeling a patient as having refractory GERD, confirm PPI adherence and timing. Studies show up to 50% of patients take PPIs incorrectly.
