Pre-Conception Counseling in IBD

The most important principle: disease remission at time of conception is the single greatest predictor of favorable pregnancy outcomes in IBD. Active disease significantly increases risk of preterm birth, low birth weight, and pregnancy loss.

✅ Goals Before Conception

Achieve and maintain clinical, endoscopic, and/or biochemical remission
Optimize nutrition: folate 400 mcg/day (start 3 months before conception)
Vitamin D, iron, B12 repletion as needed
Review and rationalize all medications pre-conception
Stop methotrexate ≥3–6 months before conception (Strong rec)
Discuss realistic pregnancy risk based on disease type and activity
Multidisciplinary care: GI + OB ± MFM for high-risk patients

⚠️ Pregnancy Risks in Active IBD

Preterm birth (2–3× increased risk)
Small for gestational age / low birth weight
Spontaneous pregnancy loss
Cesarean delivery (especially UC/Crohn's with perianal disease)
Venous thromboembolism risk is elevated in IBD — highest peripartum
Neonatal adverse outcomes correlate with maternal disease activity

Fertility in IBD

Women with IBD in remission have near-normal fertility. However, active disease, prior surgeries (especially IPAA), and certain medications (sulfasalazine causes reversible male infertility — switch to mesalamine before conception) can reduce fertility. Voluntary childlessness is also high in IBD patients due to fear — counseling is essential.
